Please print the sheets from the pdf file below( only for Chiropractic ), or recieve them by fax,
or just come to our clinic 20 minutes before your reservation time to fill in.

2.We prepare gown and pant for you to take chiropractic treatment.

3.We ask treatment fee in cash before the session. Credit cards are not available.

Mondays,Tuesdays, Wednesdays, Thursdays, Saturdays. Sundays

9:30〜12：30　　14：00〜18：30

Fridays　　14：00〜20：00

Dr.Hiroshi Yamada will be giving treatments until 8p.m. on weekdays.

※You can share the ticket with your family

Bio-energy Therapy

Long distance session fee is the same as above.

Chiropractic department offers to complete your insurance claim forms without charge.
